Registered Nurse - Client Care Liaison Team Leader

Are you a motivated and experienced Registered Nurse with a passion for supporting older Australians? We are seeking a dedicated Office Based Registered Nurse to join our Home Care Package (HCP) Aged Care Team. In this pivotal role, you will provide clinical oversight, manage a team of Enrolled Nurses, and ensure effective delivery of high-quality care across our HCP Program.
Responsibilities
- Provide leadership, supervision, and professional support to a team of Enrolled Nurses.
- Oversee the day-to-day management of the HCP program, ensuring compliance with the Aged Care Quality Standards.
- Conduct clinical assessments, review care plans, and monitor outcomes to ensure person-centred care.
- Collaborate with clients, families, GPs, and allied health professionals to support best practice.
- Drive quality improvement initiatives and maintain accurate clinical documentation.
- Assist the business and Aged Care team to transition to Support at Home.
About You
- Current APHRA registration as a Registered Nurse.
- Demonstrated experience in aged care, community care or HCP management.
- Strong leadership skills with the ability to mentor and manage a team of nurses.
- Excellent communication skills, problem solving, and organisational skills.
- Sound knowledge of Aged Care legislation, compliance and reporting requirements.
- Proficiency in care management systems and Microsoft Office Suite.
