Job Title: Support Coordinator
We are seeking an experienced and dedicated professional to join our team as a Support Coordinator. In this role, you will play a vital part in helping NDIS participants achieve greater independence by assessing their individual needs and identifying the necessary services and supports to reach their goals.
About the Role:
* Assessing participant needs and developing personalized plans to achieve their goals;
* Closely working with participants, families, and external service providers to coordinate support services;
* Maximizing participant NDIS plans through effective resource allocation;
* Avoiding barriers by implementing action plans that promote growth and development;
Your Key Responsibilities:
o The building of strong relationships with clients is paramount
o You may be required drive around town during business hours within normal circumstances although currently less frequently than before throughout pandemic. extbf{Use My Car Allowance}
o This could involve discussion with Service Providers so they understand all aspects including budgets allocated ,and he appropriate selection best suited for individuals client requirements especially budget allowance .As well You must work closely targets hourly bills completed on time staying ahead target completions (hopefully sooner!).
