The Aged Care Assessment Service (ACAS) South provides comprehensive assessments for older Tasmanians.
This position plays a vital role in delivering high-quality, person-centred care that supports our clients in accessing appropriate government-funded aged care services.
We are seeking an experienced Aged Care Assessor to conduct Home Support Assessments through the My Aged Care platform.
* Conduct thorough Home Support Assessments using the Integrated Assessment Tool (IAT).
* Identify and evaluate an older person's needs, goals, and preferences, focusing on wellness, reablement, and promoting independence.
Key Responsibilities:
* Develop and implement individualised service plans to meet the needs of older clients.
* Coordinate with multidisciplinary teams to ensure seamless client care.
* Utilise funded and non-funded services to achieve optimal outcomes for clients.

Requirements:
* Demonstrated understanding of the needs of older persons and ability to plan and coordinate client care independently.
* Strong organisational skills and ability to meet deadlines.
We offer a competitive salary range of $81,915 to $94,263 per annum, plus superannuation.
A current driver's licence and experience in case management, aged care, or support work are highly desirable.
