About the Role
You will be conducting face-to-face assessments with older Australians to determine eligibility for supports under the My Aged Care program, therefore must be willing drive to clients homes. You'll help clients access services to stay independent in their homes and communities.
You will work autonomously within your region and report to the Assessment Team Lead.
Key Responsibilities:
* Conduct client assessments in their homes using a wellness and reablement approach
* Develop person-centred support plans
* Review and monitor ongoing client needs
* Liaise with referral agencies and community service providers
* Maintain accurate records using the My Aged Care portal
About You
We're looking for compassionate, self-motivated professionals who thrive working independently in the community.
You will have:
* Degree in Allied Health, Social Work, or a related field
* Registration with AHPRA or membership of a relevant professional association (AASW)
* Current Australian Driver's License and own reliable, insured vehicle (Vehicle Allowance provided under SCHADS Award)
* High level of digital and computer literacy
* National Criminal History Check & Working with Children Check (NSW or equivalent)
* Current Influenza vaccination (or willingness to update)
* Strong verbal and written English communication skills are essential for this role, alongside the ability to engage respectfully across cultures.
* Good internet and phone reliability in your home.
