The Clinical Nurse Consultant (CNC) is an advanced practice clinical leader responsible for coordinating patient-centred care within the Acute Care of the Elderly (ACE) specialty. This role focuses on providing expert clinical oversight and management for patients with dementia and delirium, ensuring, high quality, specialised care tailored to the needs of the older adult.
The CNC (ACE) identifies patients with acute illness, frailty, or cognitive impairment in the Emergency Department and ensures timely transfer to a specialised Acute Care for the Elderly (ACE) setting for early assessment and intervention, streamlining patient flow and improving care outcomes.
The CNC demonstrates sound knowledge of contemporary nursing practice and theory and provides clinical leadership, advice and advocacy to a multidisciplinary team and is responsible for the facilitation and provision of quality and cost-effective nursing and/or midwifery services which are consistent with contemporary standards, guidelines, relevant legislation and best practice.
