We are seeking a skilled professional to provide person-centred case management to clients in need. As an expert in housing systems and tenancy support, you will work collaboratively with internal teams and external services to coordinate wraparound support and enable clients to transition from short-term accommodation into long-term sustainable housing.
* Provide high-quality case management to clients accessing immediate housing responses.
* Support clients in achieving long-term sustainability through effective housing transitions.
* Develop and implement early intervention approaches to prevent homelessness and reduce tenancy stress.
* Maintain accurate records and contribute to service reporting and evaluation.
* Relevant degree or less formal qualification with substantial years of relevant experience.
* Strong understanding of housing systems and tenancy support principles.
* Skilled in managing complex caseloads and delivering therapeutic interventions.
* Professional approach with the ability to maintain clear boundaries and establish trust with clients.
* Experience working with diverse stakeholders and service providers.
* Excellent communication, administrative, and organisational skills.
