We are looking for an Experienced Clinical Nurse Manager to join our clinical team. You will work closely with the Leadership Team to develop and implement strategies that promote growth of the service, improving participant outcomes and the overall business performance.
This is a full-time role Monday - Friday.

This role will lead the strategic, operational and clinical delivery of our In-Home Nursing Services to our participants.
Duties will include but are not limited to;
Provide would care, catheter management, palliative care, chronic disease support and post-acute care and overseeing the daily operations and delivery of clinical services within a community setting
Hands on approach ensuring all participants receive person-centred care in line with the Aged Care Act and Aged Care Quality Standards
Supervision of nursing staff and managing resources to ensure safe and effective participant care
Clinical leadership, providing guidance and support to the organisation
Monitor and evaluate the quality of care, care planning, quality indicators and identify areas for improvement
Oversee systems for clinical escalation, restrictive practices authorisation and risk identification
Be a clinical leader, providing guidance and support to the organisation, and promote professional development
Implementing and Evaluating policies and procedures related to clinical services
Conduct comprehensive nursing assessments and ongoing clinical reviews

In order to success in this role you will have the following;
* Current Registration as Registered Nurse with AHPRA
* Minimum 5 years post graduate, clinical experience
* Previous Leadership experience, mentoring and supporting diverse clinical teams
* Required to possess a broad range of clinical skills
* Demonstrated experience in the community, aged care or similar nursing roles
* Strong clinical assessment, decision making and documentation skills
* Solid understanding of compliance within regulated environments (Aged Care Quality Standards)
* Business experience such as risk management, budgets, KPI's and strategy implementation
* An understanding of person-centred care and a commitment to work within Southcare's purpose and values
* National Police Clearance in line with the requirements of the Aged Care Act
