Job Title: Project Officer Multidisciplinary Care
Job Description:
The Western NSW Primary Health Network (WNSW PHN) is seeking a motivated individual to join their team as a Project Officer Multidisciplinary Care. This role will involve guiding and facilitating effective engagement with the allied health sector in western NSW PHN region to strengthen multidisciplinary team-based care in primary care.
This position will provide leadership on coordination, advocacy, education and establishment of allied health partnerships with General Practices and ACCHOs to influence and support implementation of evidence-based care.
The successful candidate will be guided by the 10-Year Primary Health Care Plan and the National PHN Allied Health in Primary Care Engagement Framework to strengthen their engagement with the allied health sector within the WNSW PHN region.
Key Responsibilities:
* Build effective relationships with clinicians, stakeholders and communities to understand key challenges and opportunities to strengthen multidisciplinary team-based care in primary care settings
* Utilize critical enablers such as digital health, continuous quality improvement activities, education and commissioning to achieve outcomes
* Develop project reports, management briefings and project materials to support the delivery of the role's objectives
* Manage project timelines and coordinate stakeholder engagement to deliver high-quality outcomes
Requirements:
To be successful in this role, you will need:
* Experience working in and/or knowledge of primary care, including General Practice, Aboriginal health services, allied health services, community pharmacy or Community Health services
* Excellent interpersonal skills, with well-developed written communication skills
* Experience in project management, including creating and managing project timelines
* Demonstrated understanding of Aboriginal culture, including a commitment to cultural awareness and safety
* High-level interpersonal and communication skills, with high-level competence in using information communication technology
* Ability to travel within western NSW, with a current driver's license
