Urgent Care Team Leader

We are seeking an experienced and confident Registered Nurse to join our Urgent Care team as the Urgent Care Team Leader. In this role, you will ensure patients presenting to Urgent Care are promptly assessed, triaged accurately, and managed safely in collaboration with the Urgent Care Medical Practitioner. You will support clinical operations, guide Urgent Care nurses, and uphold ForHealth's policies, standards, and service excellence.

About the Role

You will provide frontline clinical care, support patient flow, and act as a key liaison between the nursing team, medical practitioners, and practice leadership. Working closely with the Practice Nurse Manager and Practice Manager, you'll play an integral role in maintaining a safe, efficient, and patient-centred Urgent Care environment.

Key Responsibilities

Triage & Clinical Assessment

* Conduct comprehensive triage and escalate high-acuity presentations promptly.
* Review referral handovers and determine appropriateness for Urgent Care.
* Initiate care interventions relevant to patient presentation.
* Provide timely handover to the UC practitioner.

Direct Patient Care

* Deliver patient care activities as directed by the Health Care Practitioner.
* Perform wound care, plastering, suture removal, and post-procedure education.
* Assist with clinical procedures, diagnostic tests, and vaccinations.
* Cannulate and administer IV antibiotics/fluids within scope.
* Provide phlebotomy and catheter insertion/change/removal (if applicable).
* Prepare and provide take-home medications as required.

Clinical Administration

* Maintain accurate and comprehensive clinical documentation.
* Manage UC stock ordering and ensure vaccines/medications are in date.
* Oversee equipment maintenance and escalate repairs.
* Ensure follow-up referrals and documentation are provided on discharge.
* Update AIR immunisation records.

Workplace Health & Safety

* Adhere to WHS policies and safe work practices.
* Report incidents and hazards within designated timeframes.

People & Patient Management

* Support PM/PNM with onboarding, training, and clinical guidance.
* Model best-practice triage and clinical care for UC nurses.
* Build rapport with patients and ensure a smooth, efficient patient journey.
* Escalate patient concerns or breaches of policy to ensure safe care.

About You

* AHPRA Registered Nurse (RN).
* Minimum 5 years post-graduate nursing experience.
* At least 1 year Emergency experience within the last 5 years.
* Proven triage experience in an emergency setting.
* Primary care experience desirable.
* Strong critical thinking, problem solving, and communication skills.
* Ability to work in a fast-paced environment with competing priorities.
* Excellent documentation, organisation, and time-management skills.
* Strong team collaboration and ability to coach/support others.
