* Work closely with occupational therapists to develop tailored reablement plans that support customers in achieving their goals.
* Prioritise the health, safety and wellbeing of customers and colleagues alike.
* Engage in ongoing professional development, assuming responsibility for your own training and skills enhancement.
* Assist customers with the safe use of mobility aids and assistive equipment as required.
* Deliver high-quality reablement support in various settings, including customer homes, day centres, hospitals and community locations.
* Support customers with physical transfers, adhering to health and safety protocols and organisational policies.
* Contribute to effective communication, accurate record-keeping and incident reporting within customer records.
* Participate in regular team meetings and review sessions.
* Maintain confidentiality and adhere to data protection guidelines at all times.
* Foster strong relationships with colleagues and other healthcare professionals through open communication.
* Attend supervisions, appraisals and objective setting exercises.
* Offer flexible support to the team when needed, encompassing tasks such as filing, answering calls and assisting with administrative duties.
