We are seeking a skilled Transition Support Specialist to join our team. This role is crucial in providing a collaborative approach, engaging and supporting Young People from 15 to transition successfully out of care.
About the Role
The Transition Support Specialist will help develop young people's skills and knowledge for independent adult life, delivering support until age 25.
Key Responsibilities
* Address the impacts of abuse, neglect, separation from families, and complex trauma on young people in out-of-home care and adults who have exited the care system.
* Utilise Therapeutic Crisis Intervention techniques to assist in behaviour de-escalation and help young people develop coping strategies.
* Develop comprehensive plans for each young person to support their transition to independent living and engagement with community supports.
* Work with Child Safety staff to support young people in care aged 15+ in developing a Transition and Financial plan, gaining necessary skills, knowledge, and networks.
* Facilitate opportunities for young people to participate in peer groups and networking, enabling them to develop relationships, learn from others, and build a social support network.
* Develop and maintain relationships with significant family members, friends, community organisations, and government agencies to promote lasting connections and positive outcomes for clients.
* Incorporate strategies within assessments and goal plans to help young people maintain their culture and identity.
* Support and promote engagement with local Aboriginal and Torres Strait Islander Communities and Community Groups.
