We are looking for a skilled and motivated Nurse to become avital part of our healthcare team. This role involves working closely alongsideGPs, Advanced Nurse Practitioner, the Nursing Team, and administrative staff toprovide outstanding patient care. The successful candidate will demonstratestrong clinical knowledge, effective communication abilities, and a proactiveapproach to enhancing patient health and wellbeing. Ideally, we would likesomeone with an interest in Diabetes Care, but training can be provided.

Main duties of the job

Key Responsibilities:

Help manage chronic diseases- including diabetes, asthma, COPD, hypertension, and cardiovascular disease. A particular interest in diabetes would be advantageous.

Administer immunisations safely and efficiently.

Perform cervical smears, wound care, and various treatment room procedures.

Provide family planning advice and support tailored to patient needs.

Promote health initiatives such as smoking cessation and weight management programs.

Conduct routine health checks and patient reviews to monitor wellbeing.

Maintain accurate and up-to-date patient records while following practice protocols.

What We Are Looking For:

Registered Nurse with the NMC, ideally with experience in a primary care setting.

Skilled in managing chronic conditions and proficient in treatment room procedures.

Dedicated to delivering patient-centred care with a focus on clinical excellence.

Able to work independently as well as collaboratively within a multidisciplinary team.

Familiarity with SystmOne or similar clinical systems is advantageous but not essential.

Committed to ongoing professional development and learning.

Our practice offers a comprehensive range of primary care services designed to meet the diverse needs of our patients, including chronic disease management, preventive care, health promotion, family planning, and same-day urgent appointments. As part of a local Primary Care Network (PCN), we collaborate with other healthcare providers across the South Hams to deliver integrated and seamless care. This partnership allows us to share expertise and resources, improving access to specialist services and enhancing overall health outcomes within the community.

Our experienced team includes 5 GP Partners, 4 Salaried GPs, 1 Advanced Nurse Practitioner (ANP), 3 Practice Nurses, 1 Nursing Associate, 3 Healthcare Assistants (HCAs), and 2 Phlebotomists. We are also supported by a dedicated reception and administrative team. Together, we are committed to providing high-quality, accessible, and compassionate healthcare in a supportive and friendly environment.

Job PurposeWe are seeking an experienced and enthusiastic Practice Nurse to join our team. The successful candidate will work collaboratively with GPs, the ANP, nursing colleagues, and support staff to deliver exceptional patient care and improve health outcomes.

Key Responsibilities

Manage chronic disease clinics such as diabetes, asthma, COPD, hypertension, and cardiovascular disease.

Administer immunisations following practice and national guidelines.

Undertake cervical smears, wound care, and other treatment room procedures.

Provide family planning advice and support.

Support health promotion initiatives, including smoking cessation and weight management programs.

Conduct routine health checks and patient reviews.

Maintain accurate patient records and adhere to practice protocols and standards.

Person Specification

Registered Nurse with the Nursing and Midwifery Council (NMC), preferably with primary care experience.

Proficient in chronic disease management and treatment room procedures.

Strong commitment to patient-centred care and clinical excellence.

Ability to work independently and as part of a multidisciplinary team.

Experience with SystmOne or similar clinical systems is desirable but not essential.

Evidence of ongoing professional development.
