As we continue to grow, we are seeking a highly skilled and experienced Clinical Nurse Specialist to join our team and lead the delivery of complex clinical care in the community.
What We're Looking For
* Candidate who can commit to a minimum 20-hour work week, plus the option for additional hours
* Current AHPRA registration as a Registered Nurse
* Minimum 3 years of clinical nursing experience, with demonstrated capability in community or aged care settings
* Strong experience in wound care, catheter care, and complex case management
* Proven clinical leadership and mentoring skills
* A current driver's licence and access to a reliable vehicle
* Current National Police Check or willingness to obtain
* Unrestricted work rights in Australia"

Key Responsibilities
Complex Wound Care
* Conduct thorough wound assessments (including photography, measurements, and staging)
* Develop and implement individualized wound management plans
* Evaluate dressing types, advanced therapies, and negative pressure wound options
* Insert, maintain, and troubleshoot indwelling (IDC) and suprapubic (SPC) catheters
* Educate patients and carers on proper catheter care, infection prevention, and early signs of complications
Patient Assessment & Care Planning
* Perform holistic in-home assessments (covering physical, psychosocial, and environmental factors)
* Develop and review person-centered care plans in collaboration with allied health and medical teams
* Coordinate referrals to relevant health and community services
Clinical Leadership
* Mentor and support junior nurses and student placements in the community
* Lead and contribute to the development of clinical policies, procedures, and best-practice guidelines
* Participate in clinical audits and quality improvement initiatives
