ResponsibilitiesProvide expert clinical consultancy to patients, carers, and healthcare professionals, supporting best practice care for older persons with complex health needs.Work in collaboration with the Nursing Unit Manager, medical teams, Allied Health, and support staff to implement harm-minimisation strategies following geriatric assessments.Deliver specialist consultation and develop management plans for patients with cognitive impairment, including those presenting with challenging behaviours.Advocate for and support holistic, person-centred care that respects individual patient needs and rights.Facilitate collaboration with interagency and community liaison services, including Residential Aged Care Facilities and private providers.Lead the development and review of policies and procedures relating to older person care across Maitland Hospital and the broader Hunter New England Local Health District.Collate and report on older person care data, ensuring timely submission to Local Executives, HNELHD, and the Ministry of Health.Act as a senior nursing resource, supporting knowledge transfer, mentorship, and the career development of less experienced practitioners and health care team members.
Qualifications and ExperienceRegistered Nurse with current Authority to Practice with AHPRA with at least 5 years full time equivalent post-registration experience.Approved post-registration nursing qualifications in Older Persons.An experienced senior nurse with advanced clinical expertise in the care of older persons.A skilled communicator who can work collaboratively with patients, families, and multidisciplinary teams.Passionate about improving outcomes for older people and committed to promoting holistic, rights-based care.Experienced in policy development, data reporting, and leading service improvement initiatives.A natural mentor and leader, dedicated to supporting and developing the capability of others.
