Holistic Horizons Support Services is seeking an experienced Nurse Practitioner to join our multidisciplinary team in a contractor capacity, providing clinical care to participants across our NDIS disability services and aged care support programs.
This role suits a practitioner who enjoys working autonomously in the community, while collaborating with a supportive care team. You will deliver advanced clinical care to individuals with complex needs while operating under your own Medicare billing arrangements.
This is an excellent opportunity for a Nurse Practitioner looking to expand their community-based practice with an established referral network.

We are now also delivering aged care services through the Support at Home framework, enabling us to provide holistic support across the lifespan. Our team works closely with families, support coordinators, allied health professionals and healthcare providers to deliver person-centred care that empowers individuals to live well in their homes and communities.
About the Role
As our contracted Nurse Practitioner, you will provide advanced clinical assessment, primary care and health management for participants receiving disability and aged care supports.
You will work alongside our service delivery team while maintaining clinical autonomy and independent Medicare billing.
Key responsibilities include:
* Conduct advanced health assessments and clinical reviews
* Develop and implement client care and management plans
* Provide primary care support for NDIS participants and aged care clients
* Review medication charts and clinical documentation
* Collaborate with support coordinators, families, allied health and GPs
* Provide clinical guidance to support staff where required
* Contribute to quality improvement and best-practice care delivery

This role is offered as an independent contractor position.
Operate under your own Medicare provider number
Manage your own billing through Medicare
Flexible working hours based on client demand

Endorsement as a Nurse Practitioner with AHPRA
Current NDIS Worker Screening Check (Yellow Card)
Working With Children Check (Blue Card)
Current Driver's Licence

Ability to work autonomously within community settings
