Renal Care Coordinator Job Description
We are seeking a highly skilled Renal Care Coordinator to join our team at Central Australian Aboriginal Congress. The successful candidate will be responsible for coordinating renal care services, working closely with patients, families, and healthcare teams to ensure the best possible outcomes.
Duties and Responsibilities:
* Coordinate renal care services, including managing patient referrals, scheduling appointments, and communicating with healthcare teams.
* Develop and implement individualized care plans in consultation with patients, families, and healthcare providers.
* Assess and monitor clients with Chronic Disease, providing education and support to promote healthy lifestyle choices.
* Educate patients and their families about chronic disease management, medication adherence, and other relevant health issues.
* Collaborate with multidisciplinary teams, including doctors, nurses, social workers, and other healthcare professionals to provide comprehensive care.
* Ensure cultural safety and awareness when interacting with patients and their families from diverse backgrounds.
* Communicate effectively with patients, families, and healthcare teams, both verbally and in writing.
Requirements:
* AHPRA registration with at least 2 years post-graduate work experience.
* Excellent knowledge and understanding of Primary Health Care principles.
* Commitment to working within Aboriginal Community Controlled services.
* Ability to work independently and as part of a multi-disciplinary team.
* Strong organizational skills, with the ability to manage competing priorities.
* Effective communication skills in a cross-cultural environment.
* Ability to maintain strict confidentiality.
* Fully vaccinated for covid-19 (Three doses) or willing to be vaccinated as a condition of employment.
Education and Qualifications:
We are looking for candidates with a strong educational background in a relevant field, such as nursing, allied health, or a related discipline.
