Silver Chain offers a permanent part‐time Case Coordinator role on the Sunshine Coast. The position is based out of our Birtinya and/or Gympie Service Delivery Centre and is responsible for allocating client services across the Regional Council areas of Gympie, Noosa and the Sunshine Coast. There are no night shifts.
Key Responsibilities
- Provide leadership, support and ongoing development to Care Workers and Clinical Team
- Recruit, interview and appoint staff and conduct staff appraisals
- Liaise with the CNM to ensure effective delivery of clinical services
- Coordinate and manage service delivery to meet client needs, achieve cost and volume targets, contractual obligations, policies and procedures
- Implement strategies to control human and material resource utilisation and expenditure, predict trends in patient care provision, influencing resource expenditure and prepare submissions
- Engage in resolving complex clinical problems
- Coordinate clinical team meetings
- Maintain accurate and up‐to‐date documentation and disseminate documentation
- Coordinate and support Nurses, Un‐regulated Care Workers, Student Placements, Volunteers and Credentialed General Practitioners to transfer of knowledge and work experience
- Participate in project work and special interest groups and undertake research in support of business unit objectives
- Build relationships and network with external parties to promote Silver Chain services, identify opportunities for growth
- Respond to client complaints
- Manage occupational health and safety within legislative requirements and organisational policies and procedures to minimise risk for employees and clients to ensure a secure and healthy work setting
About you
- Current registration with AHPRA with demonstrated high level clinical skills and understanding of care delivery requirements within the community
- Experience in the management of physical and financial resources
- Experience in leading a team and coordinating resources
- Demonstrated time management and organisational skills
- Well‐developed communication and interpersonal skills, with capacity to negotiate and problem solve
- Demonstrated empathetic and flexible approach to manage sensitivities of client and employee issues within a clinical environment
- Demonstrated ability to work autonomously with a team focus to contribute to business unit and organisational objectives
- Flexible approach with decision making, negotiation, influencing and advanced problem‐solving skills to manage allocation of resources and resolve complex clinical problems
- Demonstrated analytical skills to identify risks and areas for improvement and make recommendations for change
- A certificate IV in Training and Assessment is desirable
- Completion or commencement of work toward relevant post‐graduate studies
