Key Role Summary: Our dedicated team is seeking a passionate and compassionate support worker to provide high-quality care and support to individuals with disabilities in their homes and communities.
This rewarding role involves assisting clients with daily living tasks, community participation, and skill development, while promoting their rights, choices, and well-being. As a key member of our team, you will work closely with clients, families, and other professionals to deliver holistic, high-quality care aligned with our values and the NDIS Practice Standards.
* Assist clients to engage in age-appropriate tasks and activities, including in-home support, community access, and employment.
* Provide direct care as outlined in the individual support plan.
* Assist clients in accessing social and recreational activities.
* Monitor client well-being and promptly report concerns to management.
* Uphold professional conduct aligned with our values and legislative requirements.
Role Responsibilities:

What You'll Need:
* Diploma or Certificate IV in Disability Support or related field.
* Relevant experience working with people with disabilities.
* Excellent communication and interpersonal skills.
* Ability to work independently and as part of a team.
* Basic computer skills and familiarity with software applications.
