* Main Responsibilities:
o Provide personal care and daily living assistance to the client, ensuring their safety and well-being at all times.
o Assist with medication management, adhering to prescribed dosage and schedules as required.
o Engage in activities that promote socialisation and community integration, fostering a sense of belonging and connection for the client.
o Monitor and report any changes in the client's health or behaviour, maintaining accurate records and providing timely updates to relevant parties.
o Collaborate with healthcare professionals to ensure seamless care and support, leveraging expertise and resources as needed.
* Requirements:
o Hold current qualifications in Individual Support, Disability Support, Aged Care, or equivalent.
o Demonstrate experience working with adults with physical and intellectual disabilities, showcasing adaptability and patience in a dynamic environment.
o Possess excellent communication and interpersonal skills, facilitating effective collaboration and relationships with clients, families, and healthcare teams.
o Show evidence of ability to work independently and as part of a team, prioritising tasks and managing time efficiently to meet deadlines.
o Exhibit physical fitness level allowing for active participation in activities, promoting overall health and wellbeing for the client.
