About the role
An exciting opportunity for an experienced Support Coordinator - Level 3 to join our amazing Heart2Heart Team as the Senior Support Coordinator in Bundaberg, overseeing both of our locations (Bundaberg and Toowoomba) This position is open to Part Time up to Full Time hours.

The Senior Support Coordinator will apply attentive interpersonal skills, an understanding of the NDIS, time management, and planning and administration, to enable each NDIS participant to access the supports and services they need to lead everyday lives, and to reach their personal goals.
* Provide Level 3 - Specialist Support Coordination
* Manage a complex participant case load and be the Subject Matter Expert (SME) portfolio holder, providing education, training, and best-practice information to the Support Coordination Team
* Provide leadership, mentoring and coaching, capacity building support, supervision on complex cases and build the capacity of others in the Support Coordination Team
* Support the Team with the day-to-day running's and promotion of Heart2Heart Organisation across both locations
* Conduct initial assessments of participants, interpreting NDIS plans and health information, as well as personal circumstances and manage caseloads between the Team
* Promote empowerment, choice and control and community inclusion of people with disabilities and their families, consistent with their NDIS plan
* Ability to attract and retain the targeted population of NDIS customers
About You
The successful applicant will have excellent time management skills, flexibility and willingness to grow with the organisation as well as;
* Minimum 1-2 years experience in Support Coordination (preferred)
* Successfully demonstrated experience in coordinating responsive, individualised support to participants with complex needs
* Qualified and experienced Psychologist, Occupational Therapist, Social Worker, or Mental Health Nurse encouraged to apply
* Detailed working knowledge of relevant community networks and relevant external services
* A strong passion for helping others to learn and grow, a positive attitude, show initiative, are dedicated, and have a professional and personalised presence
* Adaptability and ability to contribute in a positive and proactive manner within a multidisciplinary team
* Empathy and understanding for clients and staff while maintaining emotional resilience
