Key Responsibilities:
* Conduct comprehensive functional assessments on admission and periodic reviews to ensure optimal care.
* Develop tailored treatment plans that cater to the unique needs of each client.
* Deliver therapy services in both one-on-one and group settings to promote physical independence and overall well-being.
* Actively participate in referrals, post-fall reviews, and multi-disciplinary team meetings to ensure seamless collaboration and high-quality care.
* Provide manual handling training to maintain a safe and supportive work environment.
* Accurately document treatment planning and interventions to inform future care decisions.
* Lead and supervise Allied Health Assistants to optimize teamwork and achieve exceptional outcomes.
