Located at either our Bundaberg or Hervey Bay offices, the Healthy Ageing Coordinator (Wide Bay) delivers Country to Coast QLD's (CCQ) healthy ageing programs, projects and initiatives across the Wide Bay area.
In consultation with the Directorate, you will partner with local stakeholders to enhance healthy ageing and person-centred health and ageing services. Essential to this role is the confidence and capacity for building productive relationships with stakeholders across acute, residential, primary health care and community services to make impactful outcomes.
Key responsibilities
* Partner to deliver and advocate for healthy ageing initiatives at a local level in collaboration with aged care, primary health and acute health services, guided by the CCQ health needs assessment, strategies, and operational plans
* Builds comprehensive knowledge of healthy ageing concepts, the aged care system, contemporary evidence-informed practice, and the role of PHNs as commissioners and quality improvement
* Implement stakeholder relationship management plans to build meaningful engagement with aged care, primary health, and acute health services on behalf of CCQ
* Build and strengthen stakeholder confidence and capability through meaningful engagement
* Assist with driving improved digital health adoption amongst aged care and primary health services, inclusive of improved electronic communication between providers, through the implementation of quality improvement projects and commissioning activities
* Work collaboratively with key stakeholders to build and deepen relationships, implement regional programs, projects and initiatives
* Contribute to integrated communication and resource activities e.g. meetings, electronic resource libraries, linkages to key local and state and commonwealth resources etc.
* Contribute to the development, implementation, promotion and evaluation of initiatives that increase community knowledge and understanding of aging and access to healthy ageing initiatives and services
* Influence and promote an ethos of safety and inclusivity for all people focusing on specific issues of older people in all cultures and cultural safety for Aboriginal and Torres Strait Islander people
* Work proactively with stakeholders to provide support during emergency events and associated activities e.g., vaccination programs, links to recovery resources, extreme weather event responses etc.
Key Selection Criteria
* 5+ years in a health-related discipline with tertiary qualifications relative to the role.
* Demonstrated experience in residential or community aged care or general practice.
* Health System Knowledge - Strong understanding of aged care and primary healthcare including general practice business and operational environment. Understanding of quality improvement, digital health and the links between primary healthcare, the broader health sector and government policy and priorities for reform.
* Delivers Results - progresses organisational priorities through data and evidence driven planning, delivery and evaluation of health service reform programs and projects.
* Demonstrated ability to support reform and change - assists others to address emerging challenges and generates support for change initiatives; identifies barriers to change and implements strategies to address these.
* Stakeholder engagement and partnership - demonstrated experience in building partnerships, networks and linkages with diverse stakeholders in a complex environment; overcoming barriers to collaboration and cooperation; formulating effective cross-sector responses to health needs and building commitment to the achievement of shared goals.
* Commitment to working with and engaging with Aboriginal and/or Torres Strait Islander people, organisations, and communities in a culturally safe and respectful manner.
* Knowledge of and commitment to Work Health and Safety.
