The Role
An opportunity has become available for several Pharmacists - Advanced (Discharge Navigator) to join Gold Coast Health (GCH) where the focus of our 12 000-strong team is on providing world-class health care to people living in the Gold Coast, and northern New South Wales region of Australia.
You will
* Deliver advanced coordination, management and clinical practice support across GCHHS
* Liaise closely with inpatient unit team leaders and other healthcare professionals to facilitate timely discharge medicine information
* Organise medication review services with primary care doctors and pharmacists
* Conduct and coordinate evaluation processes and research relating to the outcomes of this role model of care
About you
To be successful in this position you will require:
* Tertiary degree in Pharmacy
* AHPRA Registration
* Evidence of vaccination, or be able to be vaccinated against and remain vaccinated against - Measles, Mumps, Rubella, Varicella, Pertussis and Hepatitis B
* Tuberculosis screening, to identify if the tuberculosis vaccination may be required
* Declaration of serious discipline history must be disclosed
* Criminal History Screening will be required for new employees (GCH will cover cost)

Salary
* Temporary full time and Temporary part time (several positions, up to 30/09/2026, hours negotiable)
* Base salary ranging from $5 557.50 - $5 5798.90 per fortnight, $73.12 - $76.30 per hour

The Pharmacy Service provides medication management services across the Gold Coast Hospital and Health Service (GCHHS), 7 days a week. Our team of pharmacists are assisted in their roles by support persons comprising pharmacy assistants, administration officers, purchasing and stores personnel.
