The Care Coordinator role involves:
* Building strong connections with clients and their families
* Organizing support to clients with disabilities and monitoring progress
* Acknowledging client needs and resources for health, welfare, and other facilities and services
* Maintaining accurate records according to industry standards
* Reorganizing work priorities to meet fast-paced environment demands
* Supporting clients to achieve their goals
* Conducting Client Care Assessments and Care Planning
* Supervising and managing support staff
This position requires:
* Experience in assessing and planning care needs
* Solid understanding of industry standards and regulations
* Excellent organizational, administrative, and time management skills
To succeed in this role, you must:
* Hold current vaccinations
* Be able to obtain necessary certifications and clearances
