About The Role

The Whole of Health Program support Local Health Districts and Specialty Health Networks to improve access to care through performance support, health reforms and programs to meet health system performance measures.
The Whole of Health Program is leading the Discharge Patient Flow Concierge (DPFC) strategy. The NSW Government invested $53.9 million over four years to 2027–28 to the DPFC initiative. The DPFC strategy aims to improve the discharge processes and care coordination for patients, families and hospital staff. Through identifying complex inpatients who may need enhanced discharge supports and the initiation of proactive discharge interventions early in their hospital stay.
Health Manager level 2, starting salary from $114,779- 134,809 plus Super and annual leave loading, with a 38-hour work week.
What You’ll Be Doing
The Project Officer, Discharge Patient Flow Concierge. Whole of Health Program will:
Oversee the discharge patient flow concierge strategy.Assist in the development and successful implementation of the 16 DPFC enablers.Provide support to the program management functions of the DPFC strategy.
About You
We are seeking motivated and committed individuals who can support the work of NSW Health with:
Substantial skills in managing all stages of the project cycle (planning, development, implementation, monitoring and evaluation)Relevant tertiary qualifications and/or professional experience in a health care setting.Highly developed analytical skills including the ability to analyse and interpret complex information from numerous sources and health care data bases.Ability to prepare and present complex information in a succinct and clear way to Senior Executive within Health.
