About the Role

As a Care Partner, you will deliver exceptional customer service and effective care solutions to meet our customers' needs. You will engage with customers to understand their requirements and coordinate with relevant stakeholders throughout the planning, implementation, and review stages to ensure successful delivery of outcomes outlined in their personalised care plans.

This is a part-time permanent position, working 3 days per week, as part of a dedicated and supportive team servicing the community.

We offer hybrid work arrangements, including working from home and flexible hours.

About You

* You will have demonstrated experience in case management, including developing and reviewing tailored care plans.
* You will possess a proven ability to coordinate services and support diverse groups, ensuring effective customer outcomes.
* You will have strong knowledge of community support services and funding requirements, including aged care compliance standards.
* You will be able to communicate effectively and possess high-level interpersonal skills, with experience in conflict management and negotiation.
* You will have strong organisational and time management abilities, capable of working independently and prioritising tasks effectively.
* You will have a compassionate and empathetic approach to care, with a commitment to supporting the wellbeing, dignity, and independence of older people.

A relevant post-secondary or tertiary qualification in Social Work, Human Services, Community Services, or a related field—and/or equivalent industry experience—will be highly regarded.
