This is a 48 month fixed term, full-time position commencing Term 1, **** Ashburton Drive Primary School (PS) is proud to be part of the Connected Community School trial, a four-year, Department of Education initiative that is part of the Better and Fairer School Agreement.
Commencing in ****, the trial aims to connect our students, families and school community with services and supports to improve education and wellbeing outcomes.

The Role We are looking for an experienced and highly skilled Social Worker to join our Connected Community School trial team and work together with community services, other government agencies, health providers and local organisations to build positive, trusting relationships and better connect students and their families with the supports they need.
The successful applicant will play a vital role in supporting a whole-school approach that strengthens partnerships, builds positive community relationships and supports students and their families to improve student wellbeing, learning and life outcomes.
You should have a clear understanding of contemporary professional and practice issues, legislation, and policy relating to Social Work and demonstrate knowledge of working with children and families who are at high risk.
As Social Worker, you will ideally build rapport with our school community, undertake assessment of referred students, identify suitable services and supports, prepare plans and engage in social work intervention with students and families including interviewing family members and making home visits.
You should engage in multi-disciplinary team meetings; consulting and advising other staff related to specific cases.
You will be responsible for maintaining accurate records, case notes, data and reports that support evidence-informed practice and contribute to trial evaluation.
