Job Description
We are seeking a highly skilled and compassionate Care Coordinator to join our team of healthcare professionals. As a key member of our multidisciplinary team, you will be responsible for delivering high-quality, patient-centered care to women and their families.
In this role, you will work collaboratively with our midwives, obstetricians, and other healthcare providers to plan, implement, and evaluate patient care. Your expertise in coordination and communication will ensure seamless transitions between healthcare settings, supporting the unique needs of every woman.
You will also have the opportunity to provide person-centered care, focusing on the emotional, social, and physical well-being of our patients.
Required Skills and Qualifications
* Current registration with AHPRA as a Registered Midwife (Division 1)
* Recent experience in midwifery, including completion of a new graduate Registered Midwifery program or equivalent experience
* Excellent communication and interpersonal skills, with the ability to build strong relationships with patients, families, and healthcare teams
