What you'll be doing

The Community Liaison Pharmacist Grade 3 provides advanced clinical pharmacy services to patients with chronic conditions and complex medication regimens across community and transitional care settings. The role focuses on optimising medication safety, improving patient and carer understanding, and reducing medication‐related hospital admissions through assessment, education, and coordination of care.
Working collaboratively with GPs, community pharmacists, and multidisciplinary teams, the position ensures continuity of medication supply and supports safe transitions between hospital and community. The role also contributes to staff education, quality improvement, service planning, and clinical governance, while maintaining strong links with hospital pharmacy services and supporting workforce development through supervision and education.
Selection Criteria:
* Pharmacy registration with Australian Health Professionals Regulatory Agency with no conditions on scope of practice.
* Current unrestricted NSW 'Class C' Drivers Licence (and a willingness to travel in accordance with the demands of the position).
* Demonstrated substantial Post‐Graduate experience working in a hospital setting, particularly clinical ward pharmacy.
* Excellent written and verbal communication skills.
* Computer skills, and knowledge of, and experience with Pharmacy computer systems.
* Demonstrated high level clinical skills and superior clinical knowledge.
* Demonstrated ability to work independently and manage a busy caseload.
* Demonstrated motivation to improve and develop skills relevant to current practices.
