What You'll Be Doing
In this role, you will:
* Initial Engagement & Assessment: Build rapport using a strengths-based, trauma-informed approach and conduct comprehensive biopsychosocial assessments.
* Clinical Formulation & Care Planning: Develop clinical formulations and personalised care plans tailored to individual needs.
* Risk Assessment & Management: Complete risk assessments and implement appropriate risk management strategies in line with mandatory reporting requirements.
* Referrals & Service Navigation: Connect young people to appropriate headspace services, clinicians, and consortium partners.
* Informed Consent: Clearly explain consent processes, obtain permissions, and ensure accurate documentation.
* Stakeholder Communication: Engage with families, carers, and relevant stakeholders (with consent) to support holistic care.
* Therapeutic Interventions: Deliver evidence-based individual interventions and brief targeted support.
* Group Programs & Family Work: Facilitate psychoeducation groups, social inclusion activities, and family consultations.
* Multidisciplinary Collaboration: Work collaboratively within a multidisciplinary team to provide integrated, holistic support.
