Upcoming Mergers
On 1 January **** five health services (Kooweerup Regional Health Service, Peninsula Health, Alfred Health, Bass Coast Health and Gippsland Southern Health Service) will come together to create Bayside Health.
A newly merged health service with more than 22,000 employees caring for a community of over 1.2 million Victorians.
The new health service will provide people with the best possible care with services that support them throughout their lives.
The merger will also strengthen our workforce, providing new training, education and career opportunities.
Subject to final approval on 1 January ****, all employees from the five health services will transfer to Bayside Health.

What You Will Be Doing
The Transition Care Program (TCP) at Peninsula Health sits within the Rehabilitation, Ageing, Pain and Palliative Care Services (RAPPS) cluster and provides a comprehensive range of multidisciplinary services across multiple service sites throughout the Mornington Peninsula.
TCP works with older people at the interface between hospital, the community and home aiming to minimise inappropriate extended hospital.
This position is full time limited term until June ****.

What You Need
Eligibilty for Australian Association of Social Work Membership
Current and valid Driver's Licence
Proficient in using Microsoft office Suite and computer programs
Experience in a sub-acute and/or community setting (desirable)
Experience as a Case Manager with sound understanding of aged care (desirable)
