**The role**
The NDIS Intensive Support Worker will assist consumers on their journey towards independence, purposeful coping and self-management through direct individual consumer work as well as centre based/group activities to meet the consumer's goals. The key responsibilities include:
Deliver NDIS services through centre-based, group-based and/or 1:1 supports, and to:
1. Actively promote and demonstrate ODMH values
2. Follow recovery principles.
3. Provide 1:1 and/or centre-based/group activities, to assist recovery and independent decision
- making with the people we support.
4. Maintain a good working knowledge of the NDIS support items and requirements under NDIS Standards and Guidelines.
5. Review and adjust 'Safety and Wellness Plans', and other goal-focused agreements, prior to the delivery of supports; to ensure supports are delivered safely and, in the manner agreed with the people we support.
6. Adhere to and achieve the Program's Key Performance Indicators.
7. Maintain timely, current service delivery records for the people we support, including: data, case notes, goals and outcomes information; in the Client Management System and through any other required portal or method.
8. Contribute to effective team processes and joint decision-making within a self-organising framework for the benefit of people we support and of other team members. Adhere to and achieve the team functions.
9. Supporting other team members, including in relation to:
- rostering
- safeguarding and quality
- workplace health and safety
- communication
- problem-solving
- conflict resolution
- learning and development
10. Provide support for consumers with complex and high intensity needs.
11. Exercise initiative and independence, as required
12. Consult with other team members, Coach, or Practice Lead regarding higher-order or persistent complex support issues.
13. Ensure that One Door Mental Health achieves its aims for each individual and operates in accordance with guidelines and the relevant policies and procedures of One Door including WHS, Code of Conduct, Confidentiality, Privacy and Quality.
14. In addition to the above responsibilities, perform any other reasonable duties as directed from time to time.
15. Work within ODMH policies and protocol
**What we are looking for**
- Ability to exercise personal responsibility and work as part of a self-managing team, with the assistance of the Coach or Business Manager.
2. Demonstrates values consistent with person-centred ways of working and supports the person's choice and control.
3. Works collaboratively with the people we support to deliver the support each person requests.
4. Holds a Certificate III or IV in mental health, disability or related field, where qualifications are necessary for the safety of the person we support.
5. Previous experience in supporting individuals recovering from severe and persistent mental illness
6. Sound IT competence with Microsoft programs, Client Management Systems, electronic timesheets, HRIS and ability to adapt to new systems as they are introduced
7. An understanding of trauma-informed practice
8. An understanding of the importance of working with carers and families
9. Ability to use culturally safe practices in supporting Aboriginal and Torres Strait Islander peoples
10. Ability to work with culturally and linguistically diverse peoples
11. Ability to work with people who identify as LGBTQI
