Your Nursing Care (YNC) is a nurse-led community provider delivering high-quality support services across NDIS, Support at Home, and private care. YNC operates as an associate provider, supplying skilled support workers and nurses under the clinical governance and care plans of partnering organisations.

Purpose of the Role
The Specialist Support Worker provides high-quality, person-centred support to participants with complex needs, including disability, behavioural, psychosocial, and/or high-intensity care requirements.
The role supports safe service delivery in line with:
* Individual care plans
* Positive behaviour support strategies
* Clinical guidance provided under YNC governance
Key Responsibilities1. Participant Support
* Deliver daily living supports in accordance with participant goals and care plans
* Support personal care, mobility, transfers, and community access
* Promote independence, dignity, and choice at all times
* Maintain professional boundaries while building rapport
2. Complex & High-Intensity Supports
* Implement support strategies for participants with:
* Complex behaviours
* Psychosocial disability
* Neurological or physical impairments
* Follow behaviour support plans and risk management strategies
* Assist with clinical-related tasks under delegation (where applicable), including:
* Medication assistance (as per training and scope)
* PEG feeding / enteral support
* Catheter care awareness
* Epilepsy/seizure monitoring
* Recognise and escalate clinical concerns promptly
3. Documentation & Communication
* Complete accurate, timely progress notes in ShiftCare
* Record incidents, changes in condition, and risks in line with YNC policy
* Communicate effectively with:
* Clinical team (RN/EN)
* Support coordinators
* Families and stakeholders
* Participate in handover processes and follow documented instructions
4. Safety & Risk Management
* Follow all WHS requirements and safe work practices
* Use correct manual handling techniques and equipment
* Identify hazards and report risks immediately
* Adhere to infection control and hygiene standards
5. Professional Conduct
* Represent YNC in a professional, respectful manner
* Maintain confidentiality in line with the
* Privacy Act 1988
* Work within scope of practice and seek guidance when required
* Maintain reliability in attendance and communication
Key Selection CriteriaEssential
* Certificate III or IV in Individual Support, Disability, or equivalent
* Experience supporting participants with complex needs
* Understanding of person-centred care principles
* Demonstrated ability to follow care plans and behaviour support plans
* Strong communication and documentation skills
* Current:
* Driver’s Licence and reliable vehicle
* NDIS Worker Screening Check (Yellow Card)
* Blue Card (Working with Children Check)
* First Aid & CPR
Desirable
* Experience in:
* SIL/SDA environments
* High-intensity supports
* Mental health or behavioural support
* Training in:
* Manual handling
* Medication assistance
* Trauma-informed care
* Experience working within a nurse-led or clinical governance model

Working Conditions
* Community-based role with travel between clients
* Variable shift lengths (minimum engagement applies under Award)

* Early mornings, evenings, weekends
* Short-notice shifts depending on service demand

Pay: $36.00 – $50.96 per hour
Expected hours: 2 – 28 per week
