Western Queensland Primary Health Network (WQPHN) is seeking a seasoned professional to lead the next phase of our Healthy Outback Communities (HOC) initiative.
This pivotal role connects community-based service delivery with internal integration, ensuring health and wellbeing programs are rooted sustainably into local communities and business-as-usual operations.
About the Role
As the HOC Regional Service Integration Navigator, you'll play a key leadership role in embedding the HOC system of care across WQPHN's broader strategic initiatives. You'll be at the frontline of service innovation, facilitating integration, leading a small team, coordinating stakeholders, and supporting the sustainable transition of pilot projects into long-term, community-led models.
This role requires regular travel to remote communities such as Jundah, Windorah, and Boulia, and close collaboration with internal teams, Alliance partners, and community stakeholders.
This role will have a maximum-term contract ending on 30 June 2026, with potential for ongoing employment.
Key Responsibilities
Integrate the HOC model across WQPHN program areas by developing practical frameworks, risk registers, and post-implementation reviews
Provide on-the-ground leadership, supporting Wellbeing Connectors to deliver culturally appropriate and community-aligned services
Co-design health and wellbeing solutions in partnership with community members, providers, and cultural leaders
Oversee the operational transition of HOC initiatives and track outcomes through data and community feedback
Plan travel calendars to deliver timely, on-site support and facilitation.
Prepare briefings, reports, and contribute to cross-sector collaborations
Build the capability of internal teams to sustain HOC outcomes.
About You
Our ideal candidate will have a deep understanding of the Western Queensland health and social services landscape, with experience in a Primary Health Network or similar setting. You're a strategic thinker who values innovation and brings resilience, adaptability, and strong leadership to your work in remote and regional environments.
You will also bring:
Tertiary qualifications in public health, social science, allied health, or a related discipline
Demonstrated project leadership in health, mental health or community settings
Experience embedding pilot initiatives into sustainable operational services
Strong cultural capability and the ability to work respectfully with Aboriginal and Torres Strait Islander communities
Excellent stakeholder engagement, communication, and team leadership skills
High-level documentation, problem-solving, and organisational skills
Proficiency with Microsoft Office and cloud-based collaboration tools
A current driver's licence and willingness to travel regularly to remote areas.
