Job Description
This role involves providing nursing resources, education, and research support to facilitate high-quality patient care in a Transition Care Service setting.
The service operates on a hub-and-spoke model, covering Townsville and surrounding areas. The service provision aligns with Schedule A of the Commonwealth Transition Care Programme Guidelines.
Patient referrals are primarily made via SNAP or ACAT approvals. In collaboration with Geriatric Medicine, the team oversees the clinical management of non-acute patients admitted to the virtual ward.

Required Skills and Qualifications
Aged Care Pre-Employment Screening: This position requires employees to have a current National Police Certificate. Queensland Health will facilitate applicants obtaining this certificate.
Drivers Licence: This position requires the incumbent to operate a class C motor vehicle. An appropriate license endorsement is required to operate this vehicle.
Qualification: Bachelor of Nursing or equivalent certification relevant to the position is mandatory.
Multidisciplinary positions require possession of a tertiary degree or equivalent from a recognised tertiary institution.
