Registered Nurse – Home Care

Summary of Position
We are seeking a passionate and dedicated Registered Nurse to join a leading not-for-profit aged care provider in Ballarat. You will deliver high-quality clinical care and support to clients in their homes, promoting independence, wellbeing, and a better quality of life.
Key Responsibilities
* Conduct comprehensive health assessments and develop personalised care plans for clients.
* Administer medications, provide wound care, and monitor clients' health conditions.
* Collaborate with clients, families, and multidisciplinary teams to deliver holistic care.
* Educate and support clients and families to promote health and wellbeing.
* Maintain accurate, up-to-date clinical documentation in line with Aged Care Quality Standards.
* Identify and manage risks, escalating issues when necessary.
* Contribute to continuous improvement initiatives within the Home Care service.
Requirements
* Current AHPRA registration as a Registered Nurse.
* Experience in Community Care or Home Care settings.
* Endorsement to conduct Continence Assessments and prescribe continence aids (highly desirable).
* Proven ability to assess, implement, and evaluate individualised care and support plans.
* Strong leadership skills, with the ability to guide and mentor others.
* Experience educating clients, families, and care staff.
* Proficient in using the My Aged Care portal.
* Registered and insured personal vehicle.
