Role Overview
Join a dynamic team delivering person‑centred care under the Consumer Directed Care model, where you'll help empower clients to maintain independence and choice in their daily lives.
As a Community Support Worker, you'll provide essential support services including personal care, meal preparation, cleaning, transport, and social support to older adults, people with disabilities, and residents in aged care facilities. This role offers the flexibility to work closely with consumers, their families, and other health professionals to deliver high‑quality, responsive care tailored to individual needs.
Key Responsibilities
* Provide personal care, meal preparation, cleaning, transport and social support to older adults, people with disabilities and residents in aged care facilities.
* Work autonomously in community or residential settings, supporting consumer choice and independence.
* Collaborate with consumers, families and other health professionals to deliver responsive, tailored care.
* Engage in ongoing professional development and training opportunities.
Qualifications and Experience
* Certificate 3 in Individual Support (Ageing, Disability or Home & Community).
* Flexibility, empathy and strong communication skills.
* Ability to work independently and responsibly in a community or residential setting.
* Current driver's licence and willingness to drive; access to a reliable, registered and insured vehicle (minimum third‑party insurance).
* Contactable by telephone.
Other Requirements
* Appointment may be subject to a satisfactory Criminal History Check and appropriate immunisation requirements.
* DHS National Disability Insurance Scheme (NDIS) Worker Check.
* National Police Certificate (NPC) for employment involving unsupervised contact with vulnerable groups.
* Immunisation risk category B (indirect contact with blood or body substances).
