We are seeking a compassionate and experienced Case Manager with a strong background in Child Protection and the NDIS to support children and young people with disabilities living in out-of-home care. We are seeking a skilled and compassionate Case Manager with demonstrated experience in case management, child protection, youth work, and NDIS coordination. The ideal candidate will be familiar with trauma-informed care principles and behavioural management strategies, and possess the emotional resilience, communication skills, and critical thinking necessary to advocate effectively for children and young people in out-of-home care.
Key Responsibilities and Skills:
* Case Management & Support: Coordinate comprehensive, individualised care plans for children and young people with disabilities, ensuring their needs are met in a safe, supportive, and inclusive environment.
* Advocacy: Act as a strong advocate for young people, ensuring their voices are heard and their rights upheld.
* Collaboration: Work collaboratively with families, stakeholders, and third-party service providers to deliver cohesive, wraparound support.
* Communication & Problem Solving: Demonstrate strong interpersonal, communication, and critical thinking skills to navigate complex situations and support positive outcomes.
* Leadership & Team Management: Lead and support a team of senior support workers, providing direction, motivation, and oversight to ensure a cohesive and high-performing team.
* Quality Assurance: Oversee the implementation of best practices and ensure all care and support services align with organisational policies and industry standards.
* Staff Development: Promote ongoing professional development through training, mentoring, and a culture of continuous improvement.
* Compliance: Ensure adherence to all relevant legislation, accreditation requirements, and regulatory frameworks across out-of-home care services.
* Emotional Resilience & Team Collaboration: Maintain a calm, solution-focused approach under pressure while contributing positively to a collaborative team environment.
Skills and Experiences:
Required Skills And Experience
* Qualifications: A degree in Social Work/ Community Services and/or relevant qualifications in a human services framework
* Experience with working with children and young people within child protection, additional experience with working with children with a disability and NDIS systems
* Experience: Demonstrated and proven experience in case management or similar role, preferably within disability support or out-of-home care settings.
* Sector Knowledge: A deep understanding of best practices, regulations, and trends within the out of home care industry.
* Leadership Skills: Strong leadership abilities, with the capacity to inspire and motivate a diverse team to achieve common goals.
* Communication: Excellent verbal and written communication skills, allowing you to effectively collaborate with internal and external stakeholders.
* Organizational Skills: Exceptional organizational abilities, including the ability to prioritize tasks, manage resources effectively, and meet deadlines.
You will also need:
* Positive Notice Working with Children (blue card), or exemption,
* NDIS Worker Screening Clearance (yellow card), or exemption,
* LCS2 or the ability to obtain prior to commencing,
* Clean and current Queensland driver licence
