What You Will Bring To The Role

* Has undertaken or is willing to undertake a minimum Certificate III qualification in Aboriginal Primary Health Care or a minimum Certificate III health qualification in the relevant care area (including RPL processes).
* Aboriginality is a genuine occupational qualification; applicants will cite their Aboriginality. Information to assist you is available on the NSW Health Stepping Up site.
* Demonstrated experience with or willingness to learn relevant policies and procedures within the Local Health District and NSW Health that guide care services in Primary Care and Community Health.
* Ability to provide cultural brokerage with individuals, carers, families and communities; provide cultural guidance to the team; and facilitate respectful and culturally safe access.
* Experience in providing social support for people with chronic and complex care needs in collaboration with multidisciplinary teams.
* Understanding of health and psychosocial issues for people with chronic conditions; ability to communicate well with Aboriginal people, families and communities, and network with other services.
* Strong written and oral communication and computer skills.
* Current NSW State driver's licence (including P1 or above) and willingness to use for work, including driving long distances across the LHD.
