About The Opportunity

Fun, friendly, very supportive team with Flexible Work Practice available – including working from home, a home visiting model with a range of clients meaning every day is different. Work with infants, children, and young people aged 0‑18 years and their families in a long‑term contract with a multidisciplinary team comprised of Child and Family Health Nurses and Social Workers.

Individual and autonomous practice within the safety and support of a team environment, as well as working alongside DCJ, NGOs, Youth Health PARVAN, Child and Family Health, Paediatric, Mental health and allied health teams with a benefit of travelling to various sites across the District.

What You'll Be Doing

The Out of Home Care (OOHC) Health Pathway Program is a key initiative arising out of the Keep Them Safe: A Shared Approach to Child Wellbeing 2009‑2014, which is the NSW Government Response to the Report of the Special Commission of Inquiry into Child Protection Services in NSW. The OOHC HPP works across the broad range of health services as well as with interagency service partners to improve the health outcomes of children and young people in care.

The OOHC Clinical Nurse Specialist facilitates and implements health assessments and Health Management Plans for children and young people entering OOHC within South Western Sydney Local Health District (SWSLHD). The position will work within a multidisciplinary and multi‑agency approach with key stakeholders both within SWSLHD, and externally with key government and non‑government partners including Department of Communities and Justice (DCJ), Department of Education & Communities, General Practitioners, private health care providers, non‑Government organisations, as well as children and young people in OOHC and their carers. The position has clinical, liaison, implementation and monitoring functions.

SWSLHD Primary & Community Health provides a comprehensive range of community‑based services designed to improve and maintain the health and wellbeing of individuals and communities across South Western Sydney.

These services include prevention, early intervention, assessment, acute and post‑acute treatment, health maintenance, and continuing care. The team supports diverse populations, offering specialised programs for children, young people, older adults, and at‑risk communities.

With over 800 dedicated staff, including Aboriginal Health Workers, nurses, doctors, and allied health professionals, the service ensures high‑quality care close to where people live.

How To Apply

* Current Authority to Practice as a Registered Nurse with Australian Health Practitioners Regulation Agency (AHPRA). At least 3 years relevant full‑time equivalent post‑registration experience and approved postgraduate nursing/midwifery qualifications in a field relevant to the specialty field, or such other qualification, or experience deemed appropriate by the organisation.
* Demonstrated currency of practice and competence in assessing children and their families in relation to their health and social needs including an understanding of Out of Home Care and the Children and Young Person's Act.
* Demonstrated experience and knowledge working collaboratively as a member of a multidisciplinary team and with other service providers including Child and Family nurses, Allied health staff, General Practitioners, Child and Family Health Tertiary referral services and negotiation skills necessary to build and maintain collaborative relationships with a diverse range of stakeholders and to work effectively within a multidisciplinary team environment.
* Applicants must possess an Australian C‑Class driver's license (including P1 or P2). Those who hold a C class licence (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) licence within three months of appointment. Learner's licences are not permitted.
