The Role
We are looking for an experienced Registered Nurse to join our Melton Medical Centre to be the driving force and advocate for the local community in preventative health. This role will see you support GPs and their patients to identify and manage their chronic conditions and ultimately keep them out of hospital. Whilst CDM experience is an asset, a friendly \"can do\" and supportive nature that will fit well with our practice culture is the most important requirement of this role.
The Tasks
In this role, you will be involved in a wide variety of tasks including, but not limited to:
Prepare Care Plans and Team Care Arrangements to help patients manage their disease/sEmpower patients to increase ownership over their healthCoach patients on embedding health behaviour change principlesEnable patients to access ongoing care from a multi-disciplinary teamAssist in early identification of chronic disease by conducting health assessmentsWork with GPs to promote preventative healthcare throughout the practiceIncrease the number of patients who receive a care plan for the first timeEnsure patient’s care plans are reviewed regularlyManage administrative tasks relating to carrying out CDM appointmentThere is an opportunity for bonuses
