About the Role:

In this role, you will serve as a vital point of contact for clients and their families, providing support and guidance to help them achieve their goals.
Your responsibilities will include:
* BUILDING STRONG RELATIONSHIPS WITH CLIENTS AND THEIR FAMILIES
* CONDUCTING COMPREHENSIVE ASSESSMENTS TO DEVELOP PERSONALIZED CARE PLANS
* PROVIDING PROFESSIONAL HEALTHCARE ADVICE AND DIRECTION TO A TEAM OF HOME CARE WORKERS
* ENSURING ALL PRACTICES ARE CONSISTENT WITH POLICY AND LEGISLATIVE FRAMEWORKS
* Maintaining up-to-date client records and information
Requirements:
To be successful in this role, you must possess:
* A formal tertiary qualification in nursing or a related field
* Current registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA)
* Demonstrated knowledge and experience in the aged care sector
* A National criminal history check and Working with Children Check
* A valid First Aid and CPR certificate
