The Team:
is a state-wide interdisciplinary service that remotely supports patients to better self-manage their illnesses and chronic conditions in their own homes using virtual care technologies. The service aims to improve patient health outcomes and reduce potentially preventable hospital admissions.
This virtual care service operates as a state-wide navigation hub, providing a single point of access for home and community-based care, making it easier for consumers and clinicians to identify available care pathways in their preferred environment.

You will need:
* Current knowledge of, and the ability to apply nursing principles, procedures, and practices in patient care, in line with legal requirements and the Australian Nursing and Midwifery Board (NMBA) Registered Nurse standards.
* Sound interpersonal and communication skills, including written skills, and the ability to function effectively in a multidisciplinary team environment.
* Knowledge of continuous quality improvement (safety and quality) and the application of evidence-based practice in the practice setting.

Details of appointment: Fixed-term, casual position working as and when required, commencing as soon as possible for approximately 12 months.
Salary: $78,179 to $105,995 per annum (Pro rata). Our Employer 12% superannuation contribution is on top of this amount.
Access to the Grade 4 salary range $99,455 - $105,995 is subject to successful application for progression to Grade 4.
In addition, the following allowances calculated on the salaried incremental point may be available:
* Post Graduate Allowance of up to 10% based on qualification type
