About the Opportunity
As a Care Partner, you'll be a trusted advisor and advocate for clients, helping them navigate services and funding to remain living safely and independently at home.
You'll conduct comprehensive wellbeing assessments and create personalised care plans that reflect each client's goals and aspirations. Your ability to identify changing needs and respond swiftly—alongside clients, families, and our multidisciplinary team—will be key to delivering exceptional care.
You'll also manage client feedback, provide timely updates to stakeholders, and support operational efficiency through administrative tasks.
About You
You're compassionate, committed, and sensitive to the diverse needs of older people from all walks of life. You build rapport easily and bring warmth, energy, and enthusiasm to your work.
You're a natural collaborator who empowers others and thrives in a team environment. You're adaptable, organised, and proactive—always looking for ways to contribute, innovate, and solve problems.
Essential Criteria

Relevant Tertiary qualifications in the Human or Behavioural Sciences, Allied Health, Aged Care or Community Services Fields
Experience in client case management
National Police check
Driver's licence
