About the role
The Home Care Coordinator supports person-centered services for clients within the Home Care portfolio, ensuring smooth service delivery according to the client's assessed needs, service plan, and budget. They act as the central point of call for clients to problem solve and escalate issues.s regarding the scope of the role, please refer to the position description.
Key Responsibilities & Activities
* High Quality Person Centred Care / Health Promoting Services:
* Conduct client onboarding, reviews, assessments, and develop budgets for clients.
* Ensure client services are set up in a timely manner.
* Act as the primary contact for clients/care recipients, Hone Care Assistants, and Domestic staff within the region.
* Act as an advocate for a client/care recipient to assist with making informed decisions.
* Professional / Administrative:
* Undertake all tasks associated with service coordination, including change requests and equipment purchases.
* Complete relevant administrative tasks associated with onboardings.
* Coordinate vendors for the region.
About you
* Qualifications: Certificate III in Individual Support (Ageing), Aged Care, or Disability, or other related discipline; First Aid and CPR certification; Current Australian Driver's Licence (Class Car); and own registered and insured car.
* Experience: Demonstrated experience in client support roles and experience working with Culturally and Linguistically Diverse clients/groups, including Aboriginal and Torres Strait Islander Australians.
* Skills: Sound interpersonal and communication skills, time management skills, and proficient computer and keyboard skills, including the use of community aged care systems/processes (e.g., My Aged Care).
* Knowledge: Basic knowledge of the requirements of the Aged Care Standards and knowledge of in home aged care program guidelines and funding arrangements.
* Desirable: A Diploma of Nursing with current AHPRA registration as an Enrolled Nurse.
