Job Description:
As a seasoned healthcare professional, we are seeking an experienced Nurse Practitioner to provide clinical leadership and support to our multidisciplinary team.
Key Responsibilities:
* Deliver high-quality patient care through comprehensive health assessments and treatment plans.
* Triage referrals and coordinate with external healthcare services as required.
* Develop and implement evidence-based practices to improve patient outcomes.
* Collaborate with Registered Nurses to enhance clinical guidance and support.
* Monitor and manage patient deterioration, ensuring timely intervention and referral.
* Maintain accurate records and documentation using the Healix system.
* Work autonomously while maintaining close collaboration with the Operations Lead.

Requirements:
To be considered for this role, you will need:
* Current registration as a Nurse Practitioner in Australia/New Zealand/United States or equivalent qualifications;
* Proven experience working in aged care, preferably with a focus on senior populations;
* Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and colleagues;
* Strong analytical and problem-solving skills, with the ability to apply evidence-based practice to complex patient cases;
* Proficiency in electronic medical records and other relevant software systems.
