About the Role
The Family Support Worker will provide social and emotional support, and health education to women and family members throughout the woman's pregnancy and up to three years following the birth. The key functions of this role are to build culturally safe relationships with the families, provide education, offer support and facilitate appropriate referrals.
Duties
* Engage with BiOC families through a culturally safe and client led framework to build connection and gain an understanding of the families' journey.
* Work alongside the mother and family to identify programs and services that will improve the overall safety and wellbeing of the family.
* Facilitate evidence-based and culturally appropriate to BiOC mothers and their families in both individual and group settings as outlined by the manager.
* Work in collaboration with the family and wider team to ensure that logistical challenges of attending appointments are addressed and that mothers are informed of the importance of attending antenatal appointments.
* Ensuring continuality of care through working alongside the mother and family and in collaboration with the primary midwife and wider team.
* Utilise the NAT tools to gain a compressive understanding of the family's journey and develop individualised case plans which guide the mother's journey on the program.
* Strengthening the relationship with your assigned midwife by maintaining positive communication, participating in case review meetings both formally and informally, and sharing patient knowledge to continually improve service delivery.
* Coordinate, build and nurture productive connections and referral pathways for families- within the ATSICHS Brisbane, the Jajumbora BiOC Partnership and externally.
* Attend training and meeting as directed by the Program Manager.

Required
* Demonstrated experience in facilitating positive behaviour change with individuals, families, and community groups.
* Demonstrated ability and experience in working with Aboriginal and Torres Strait Islander communities and their leaders, respecting traditional culture, values, and ways of doing business.
* Ability to effectively engage with individuals and families.
* Ability to contribute effectively to the BiOC clinical and operational performance through provision of evidence-based care and information, and adhering to relevant workplace instructions, policies, and procedures.
* Demonstrated ability to work collaboratively in a close-knit team environment to achieve positive outcomes.
* Demonstrated organisation skills, including an ability to multi-task, plan. set and prioritise workloads to meet deadlines, and to work both independently and collaboratively to achieve outcomes.
* Demonstrated interpersonal skills with a focus on client focused outcomes.
Desirable
* Minimum Diploma level qualification in Human Services, Community Services, Psychology, Social Work or related field.
* Experience in case management.
* Experience working one on one with a medical professional to achieve program outcomes.
* Demonstrated interpersonal skills with a focus on client focused outcomes.
* Ability to identify psychosocial factors that impact on families and knowledge of referral pathways.
