Achieve Something Great. Join COPILOT, a dynamic and innovative service delivering rapid access to multi-disciplinary geriatric assessment for older people in the community. This role offers a unique opportunity to make a meaningful impact by supporting consumers to remain safely at home, reducing avoidable hospital admissions and improving patient outcomes. As a Social Worker within this team, you will provide specialist care, contribute to safe and early discharge planning, and work collaboratively to deliver high-quality, patient-centred services aligned with SWSLHD standards and the NSW Code of Conduct. If you possess the following, we\'d love to hear from you!
* Demonstrated relevant clinical experience and leadership within aged care and community-based settings.
* Proven ability to provide advanced-level psychosocial assessment, counselling, information, and advocacy using a consumer-centered framework in line with departmental policies, procedures, and guidelines.
* Strong capacity to exercise high-level independent professional judgement, ensuring clinical care is delivered within professional, organisational, legal, and ethical boundaries and reflects evidence-based best practice.
* Extensive experience in leading complex case coordination and communication with patients, families, and stakeholders, including effective liaison with internal and external care and service providers to optimise outcomes.
* Skilled in delivering counselling and therapeutic interventions to address client distress related to psychological, behavioural, relational, or social factors, including declining health conditions, with experience contributing to service development and multidisciplinary team (MDT) planning activities.
Selection Criteria
To help us assess your suitability for the role, please tell us as much as you can about yourself in response to the questions below.
* A Social Work degree which provides eligibility for membership of the Australian Association of Social Workers (AASW)
* Extensive post graduate experience working as a Social Worker with community outpatient and/or people with chronic diseases, including a demonstrated understanding of the health setting, complex health conditions and the impact on wellbeing.
* Advanced clinical knowledge and postgraduate experience and skills in psychosocial assessments and interventions to minimise psychosocial risk for patients, carers and families, including identifying and responding to Older Persons abuse and facilitation of applications to NSW NCAT.
* Demonstrated high level organisational and time management skills, along with excellent written and verbal communication skills.
* Applicants must possess an Australian C-Class driver\'s license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner\'s licenses are not permitted.
