Job Title: Permanency Support Caseworker
We are seeking a skilled and dedicated individual to join our team as a Permanency Support Caseworker. This role involves providing effective casework services for children, young people, and families in the Permanency Support Program.
About the Role:
The successful candidate will be responsible for managing caseloads, supporting foster and kinship carers, and collaborating with various agencies to ensure the best possible outcomes for our clients. A tertiary qualification in social work and experience in casework and Out-of-Home-Care is essential.
Key Responsibilities:
* Manage complex cases involving vulnerable individuals or groups;
* Serve as a liaison between government agencies, non-profit organizations, schools, healthcare providers,
* Develop close working relationships with foster/kinship carers and birth family members to support reunification/reconciliation strategies within case planning goals;
have responsibility over documenting details of every contact/case event accurately within electronic records system
