As a Clinical Care Partner, you'll be central to our vision to provide the 'best possible care and quality of life (for our residents and clients) in keeping with their cultural traditions and values.'

This blended role combines clinical leadership with direct therapeutic care, ensuring safe, high-quality services that truly make a difference. The role will also involve monitoring client outcomes, identifying changes in health status, and advocating for timely interventions. You'll build trusted relationships with clients and their families, offering education, reassurance, and continuity of care. Key Responsibilities

· Provide clinical oversight and coordinate care within the Support at Home program.

· Conduct clinical assessments to understand the physical, emotional, and social needs of the older person, identifying risks and tailoring care plans with clients and families

· Ensure all clients are treated with empathy, dignity, and respect by advocating for clients and addressing physical, clinical, emotional, spiritual, and social needs

· Champion evidence-based practice and ensure care is delivered which is of high quality.

· Contribute to service innovation, risk management, and business growth.

· Liaison with clients, families, referrers, doctors, and other health professionals to achieve coordinated outcomes that meet client goals.

· Undertake direct clinical care within your professional scope to deliver high quality, professional care to older people, that honours their cultural background, values, and preferences.

· Monitor individual budgets and guide clients on effective use of funds.

· Ensuring compliance with St Sergius Aged Care policies and procedures, maintain accurate documentation, uphold Aged Care Quality Standards, and proactively manage risks.

What you'll bring

You are a compassionate and committed professional who promotes excellence in the care of the older person. You have strong organisational skills and enjoy sharing your knowledge with other team members. You bring demonstrable commitment to our values of trust, respect, innovation, compassion and encompass.

You will also need:

* Current unrestricted registration with AHPRA as a Registered Nurse or Allied Health professional is required for this position.

* Current and unrestricted NSW Driver's License and a reliable, registered, and comprehensively insured vehicle suitable for work purposes are essential for this role

* Strong communication, highly organised with advanced problem-solving skills

* Well-developed clinical assessment skills with experience working in multidisciplinary teams

* Strong computer literacy skills (Word, Excel, Outlook, MS Teams, and CRM).

* Police certificate (not older than 3 years)

Ideally, you will also have:

* Previous experience in Home Care/Community/Aged Care

* Experience in client assessment, budget management, and relationship building in aged care

* Understanding of the Support at Home Program, Aged Care Strengthened Standards, and Aged Care Statement of Rights
